The Central Miramichi Community Health Centre (CMCHC) is an organization that provides primary health care services, illness and injury prevention, chronic disease management and community development services, using a Population Health approach. CMCHC is made up of a team of health care providers who are all about working together in a collaborative practice with community participation, and patient-centred care. The team includes physicians, nurse practitioners, social workers, dietitians, health promoters, counselors and other health care providers. CMCHC offers a range of health services including scheduled appointments with a physician or nurse practitioner, as well as a wide range of diagnostic and support services from the healthcare team.
